ALTERNATIVAS PARA PROMOVER RECRUTAMENTO ALVEOLAR: revisão

Resumo

Existem várias formas de aplicação da manobra de recrutamento alveolar, mas basicamente consiste na aplicação de altos níveis de pressão inspiratória com o objetivo de expandir os alvéolos colapsados para aumentar a pressão parcial arterial de oxigênio (PaO2), e na utilização de altos níveis de PEEP, necessários para a manutenção do ganho atingido. Objetivo: Revisar as diversas formas relacionadas à manobra de recrutamento alveolar, além de identificar as indicações, as técnicas de recrutamento alveolar, possíveis benefícios e efeitos adversos, bem como, os cuidados a serem tomados na aplicação desta manobra. Metódo: tratou-se de uma revisão bibliográfica, nas bases de dados SciELO, Lilacs e PubMed/Medline. Não havendo delimitação de tempo ou idioma, incluindo apenas publicações indexadas nas bases de dados citadas. Resultados: Existem vários procedimentos com o objetivo de promover o aumento da pressão transpulmonar, a fim de provocar a abertura do maior número possível de alvéolos e com isso melhorar a distribuição do gás alveolar, entre elas, posição prona, insuflação sustentada, insuflação gradual, suspiro e ventilação variável. Conclusão: Embora as técnicas de recrutamento alveolar sejam cada vez mais utilizadas, ainda não existem diretrizes bem definidas para seu emprego a fim de que sua eficácia seja assegurada. Contudo, sua implementação deve ser fei¬ta sob rigorosa monitorização, sedação, controle hemodinâ¬mico e realizada por equipe experiente.
