A IMPORTÂNCIA DO FARMACÊUTICO CLÍNICO NO ACOMPANHAMENTO DE PACIENTES RENAIS CRÔNICOS EM TERAPIAS DIALÍTICAS
Abstract
Chronic kidney disease is a disorder that affects about 10% of the world's population. Patients who have a low glomerular filtration rate and albuminuria should be referred to a nephrologist for renal replacement therapy (peritoneal dialysis or hemodialysis). These patients are predisposed to increased adverse effects due to the existence of comorbidities associated with renal failure, repetitive use of invasive devices and polypharmacy. In view of this, the clinical pharmacist plays an observation role in monitoring and individualized follow-up. The objective of this work is to show, through a literature review, the importance of the clinical pharmacist in the follow-up of dialysis patients. This study is a descriptive and documentary bibliographic review, based on the US National Library of Medicine National Institutes of Health (PUBMED), Scientific Electronic Library Online (SCIELO), ScienceDirect and Latin American and Caribbean Literature databases. Health Sciences (LILACS), using the following descriptors, in Portuguese: Pharmacists, Pharmaceutical Care, Renal Dialysis, Hemodialysis and, in English: Pharmacists, Pharmaceutical Care, Renal Dialysis, Hemodialyze, defined through the DECS. Note that peritoneal dialysis, detecting hemodialysis, provides greater independence to the patient. The clinical pharmacist is present in few multidisciplinary teams and is of paramount importance in the field of dialysis services. In this way, it was possible to conclude that the follow-up of the clinical clinician is essential for the detection of problems related to drugs, promotion of health education, creation of clinical protocols and monitoring of adverse reactions, thus promoting better medication follow-up and quality of life. dialysis patient.
