Demographic aging is a global phenomenon. In the state of Santa Catarina, longevity stands out, with 3.3 years above the Brazilian average. The objective of this study was to analyze the self-perception of elderly individuals living in municipalities in the Far West Health Region of Santa Catarina regarding previous illnesses, use of multiple medications, and anthropometric data. This is a quantitative and descriptive study with a unique approach aimed at elderly individuals. Data collection was carried out by Community Health Agents (CHAs) in homes through the application of a questionnaire. Data analysis was performed using absolute and relative frequencies, means, and standard deviations, with a 95% confidence interval and a sampling error of 5% (p<0.05). It was possible to observe that the region studied has an active population aging, especially females, with a predominance of 55.1% of the total participants. A statistically significant increase was observed in self-reported pre-existing conditions, with hypertension standing out at 60.4%, and polypharmacy occurring in 22% of the study population. This highlighted the need for clinical monitoring and management focused on obesity control and the promotion and prevention of health problems. Weaknesses in comprehensive care were noted, with the need for strategies for health promotion and prevention throughout aging.
